Case Manager
Location: Miami /Ft Lauderdale Area | On-site | Full-time
Nmble Medical is seeking a compassionate and organized Case Manager to support patients across South Florida. This role focuses on coordinating care, improving access to services, and ensuring patients receive the resources they need for optimal health outcomes.
The ideal candidate is proactive, detail-oriented, and experienced in navigating healthcare systems and community resources.
Key Responsibilities
Coordinate and manage patient care plans across providers and services
Conduct patient assessments and identify care needs
Connect patients with appropriate medical, behavioral, and social resources
Monitor patient progress and adjust care plans as needed
Collaborate with physicians, nurses, and interdisciplinary teams
Educate patients and families on treatment plans and available resources
Maintain accurate documentation and ensure compliance with regulations
Advocate for patients to improve access to care and services
Qualifications
Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field (required)
Active RN or LCSW license preferred
2+ years of case management or care coordination experience
Knowledge of healthcare systems, insurance, and community resources
Strong communication, organizational, and problem-solving skills
Experience working with diverse populations and underserved communities is a plus
Preferred Experience
Managed care, Medicaid/Medicare populations
Hospital discharge planning or outpatient care coordination
Electronic health record (EHR) systems
