Population Health Manager
Location: Cleveland, OH | On-site | Full-time
We are seeking a strategic and results-driven Population Health Manager to lead initiatives that improve clinical outcomes and operational performance across our partner networks. This role will focus on designing, implementing, and optimizing population health programs, with a strong emphasis on data analytics, care coordination, and value-based care models.
Key Responsibilities
Develop and execute population health strategies aligned with value-based care goals
Analyze clinical and claims data to identify trends, gaps in care, and opportunities for intervention
Collaborate with providers, care teams, and stakeholders to improve patient outcomes
Design and monitor care management programs for high-risk and chronic populations
Track performance metrics (e.g., HEDIS, quality measures, cost utilization) and drive continuous improvement
Support implementation of care coordination workflows and digital health tools
Ensure compliance with regulatory requirements and quality standards
Present insights and recommendations to internal leadership and external partners
Qualifications
Bachelor’s degree in Healthcare Administration, Public Health, Nursing, or related field (Master’s preferred)
4–7+ years of experience in population health, care management, or healthcare analytics
Strong understanding of value-based care models (ACO, Medicare Advantage, Medicaid, etc.)
Experience working with EMR/EHR systems and healthcare data platforms
Proficiency in data analysis and reporting tools (e.g., Excel, SQL, Tableau, or similar)
Excellent communication and stakeholder management skills
Ability to translate data insights into actionable strategies
Preferred Qualifications
Clinical background (RN, LPN, or similar) or experience working closely with clinical teams
Experience with risk stratification and predictive modeling
Familiarity with Cleveland-area healthcare systems and payer landscape
